This form is intended for the use of Eastman faculty only.
Use this form to report students who may be at risk of failing a course due to absence, lack of preparation, poor performance, or other difficulties. When submitted, this form goes to the Assistant Dean of Academic Affairs. Copies will be routed to the academic advisor or other faculty and administrators as appropriate.
