COMPLETING THE FORMS
The application for degree study contains a number of different forms. These forms provide specific information to the faculty of various departments. Applicants need to complete only those forms that are relevant to their degree programs.
BASIC FORMS:
All applicants must complete the following basic forms as indicated. In order to be scheduled for an audition/interview, an applicant must submit the completed application by the appropriate deadline. Only applicants whose applications are COMPLETE will be processed and scheduled for auditions/interviews. (Complete applications must contain the basic application form, supplemental materials, recommendations, and transcripts.)

Required of all applicants. Must be submitted online with application fee (or waiver request) and with the resumé and personal statement. Other supplemental materials required by the degree program or major must be submitted by the application deadline. (These forms include audition/interview information.) |
| Recommendation forms (3 forms are required of each applicant) |
All applicants must complete the top part of three recommendation forms. The remainder of the forms should be completed by the three musicians recommending the applicant. The recommender should seal the letter and recommendation form in an envelope, sign their name across the seal, and give the envelope to the applicant for submission with their materials. All recommendations must arrive in the Admissions Office with all other submitted materials by the application deadline. |
| Instrumental/Vocal Audition Repertory Form |
Applicants who will audition on an instrument must complete this form and bring it to the audition. It should not be mailed to the Admissions Office. This includes all BM, MM, and DMA instrumental applicants regardless of major. Applicants auditioning by self-prepared recording should send this form with the recording. (MA, PhD, and MA/PhD applicants do not perform an audition and do not complete a repertory form.) |

SUPPLEMENTAL FORMS:
Designed to provide the faculty of a specific department with additional information about each applicant, these forms also provide the applicant with important information. All supplemental forms must be submitted with the application form, by the application deadline.

The following candidates must submit this form with their recordings as part of the prescreening review: All applicants to the piano performance major, all applicants whose instrument is voice or cello, all graduate flute applicants, and all applicants whose major is conducting. |
| Supplemental Application for Degree Programs in Composition | All composition majors must submit this form, together with their scores. This includes BM, MA, MM, DMA, and PhD degree program applicants. |
| Supplemental Application for Jazz Studies and Contemporary Media (JCM) Degree Programs | All applicants to programs in jazz studies and contemporary media must submit this form, together with their preliminary tapes (and scores, if applicable). This includes BM, MM, and DMA degree program applicants. |
| Supplemental Application for Degree Programs in Music Education | All applicants to degree programs in music education must complete this form and submit it with the supplemental materials stated on the form. |
| Supplemental Application for Graduate Programs in Ethnomusicology, Musicology, Music Theory, and Music Theory Pedagogy | Applicants to the MA, MA/PhD, and PhD degree programs in these majors must submit this form together with the required supplemental materials. NOTE: Undergraduate theory applicants do not submit this form. |
| Supplemental Application for the Orchestral Studies Diploma in Strings | This form must be completed by MM and DMA degree program applicants who also wish to complete a diploma in orchestral studies, and by current Eastman students who wish to add the Orchestral Studies Diploma to their current degrees and majors. This form gives full information about this Diploma program, including audition details and repertory information for the orchestral audition with the Rochester Philharmonic Orchestra. |
| Supplemental Application for Majors in Piano Accompanying and Chamber Music | Applicants to MM and DMA degree programs with a major in piano accompanying and chamber music must complete this form. Audition repertory is listed on this form. |
| Supplemental Application for the Sacred Music Diploma | This form must be completed by MM and DMA program applicants who also wish to complete a diploma in sacred music, and by current Eastman students who wish to add the Sacred Music Diploma to their current degrees and majors. |
GRADUATE AWARDS:
Financial assistance to graduate students comes primarily in the form of merit-based graduate awards. Several awards have their own application or information form, as follows.

| Information for Graduate Financial Aid | All graduate applicants should read this form to find out about the process of how merit based graduate awards and need-based aid is assigned to successful candidates. |
| Graduate award No. 20: Classroom Teacher of Piano | Any graduate applicant keyboard major who wishes to be considered for a graduate award as a classroom teacher of piano must complete this form and prepare the materials indicated on the form. |
| Graduate award No. 21: Studio Accompanist | Any graduate applicant in Piano Accompanying and Chamber Music, and any keyboard major who wishes to be considered for graduate awards in studio accompanying must complete this form and submit it with their application materials. An audition for this graduate award will take place on the audition day. |
| Graduate awards Nos. 23, and 24: Choral Accompanist; Opera Coach | Any graduate keyboard major applicant who wishes to be considered for graduate awards in choral accompanying or opera coaching must complete this form and prepare the materials indicated. An audition for this graduate award will take place on the audition day. |
| Graduate award No. 39: Classroom Teacher of Aural Skills | Any graduate applicant who wishes to be considered for a graduate award as classroom teacher of aural skills must read this form. The interview will take place on the audition day. |
| Graduate awards Nos. 40 and 41: Systems Programmer, Computer Music Studio, and Electronic Music | This form provides important information for any applicant who wishes to be considered for these graduate awards. An interview with the faculty member who supervises these awards will take place on the audition day. (No specific preparation is required.) |

| Supplemental Form for Non-U.S. Citizens and Non-Native English Speakers | All applicants who are not U.S. citizens must complete this form; this includes permanent residents. In addition, all applicants whose native language is not English must complete this form, even if they are U.S. citizens or permanent residents. This form helps us to assess the applicant’s knowledge of English. |
| Teacher Preference Form | This form explains our studio teacher assignment policy. BM, MM, and DMA applicants who wish to express a preference for studio teacher should submit this information in the online application by the application deadline, or after the application has been submitted by submitting this form. |
| NASM Release Form | This form is required of all transfer applicants who are currently enrolled in a NASM (National Association of Schools of Music) school. This form must be submitted before an offer of admission with financial aid may be extended by Eastman. |
| Transcript Release Form | Required for current Eastman students applying to a new degree program. |
| Credit Card Authorization Form | Required if paying certain fees (accompanying request fee, rescheduling fee, enrollment deposit) via credit card |
